• Started with the master files of 7 US Labs
• First released in April 1995 with 6,500 codes
• Grown over time through voluntary submissions from laboratories, hospitals, and other organizations
• Version 2.24 contained 52,412 codes, of which nearly 39,000 are laboratory
• Maintained by Regenstrief Institute - Indianapolis, IN
